What Are Milia?
Milia are tiny, white or yellowish cysts that typically appear on the face, particularly around the eyes, cheeks, nose, and forehead. Unlike acne, these pearl-like bumps form when keratin (a protein found in skin, hair, and nails) becomes trapped beneath the skin’s surface rather than being shed naturally. The result is a small, firm bump that sits stubbornly under the skin.
Each milium (the singular form) is essentially a tiny pocket of keratin encased in a thin layer of skin. They’re usually about 1-2 millimeters in diameter and have a distinctive hard, round appearance.
Why Milia Aren’t Whiteheads
This is the most common confusion we see in our practice. While milia and whiteheads may look similar at first glance, they’re fundamentally different. Whiteheads are a type of acne caused by clogged pores filled with oil and dead skin cells, sitting right at the surface. Milia, on the other hand, are keratin-filled cysts that form deeper under the skin with no opening to the surface.
This is exactly why you can’t pop milia the way you might (though we don’t recommend it) pop a whitehead. There’s no pore or opening for the contents to escape, and attempting to squeeze them typically leads to frustration, potential scarring, and sometimes infection.
What Causes Milia?
Milia can develop for several reasons. In newborns, they’re extremely common and usually appear on the nose, chin, or cheeks. These typically resolve on their own within a few weeks without treatment. In adults and older children, milia can develop due to various factors including sun damage, long-term use of heavy skincare products or makeup, skin trauma from burns or rashes, blistering skin conditions, or the natural aging process that slows down skin cell turnover.
Certain procedures like laser resurfacing or dermabrasion can also temporarily increase the likelihood of developing milia as the skin heals.
How We Treat Milia
The good news is that milia are harmless and primarily a cosmetic concern. However, if they bother you, professional treatment is straightforward and effective. During an in-office procedure, we can safely extract milia using a sterile needle or small blade to create a tiny opening, allowing us to gently remove the keratin plug. This takes just minutes and leaves minimal to no scarring when performed correctly.
For multiple milia or recurring cases, we might recommend chemical peels, topical retinoids to increase cell turnover, or cryotherapy to freeze and remove the cysts.
Prevention Tips
While not all milia can be prevented, you can reduce your risk by protecting your skin from sun damage with daily SPF, avoiding heavy, pore-clogging skincare products and makeup, using gentle exfoliation to encourage cell turnover, and allowing your skin to breathe by removing makeup before bed.
If you have a blistering skin condition, working with a dermatologist to manage it properly can help prevent secondary milia formation.
